BMI's Real History (It Was Never Meant to Judge Individuals)

Invented by a mathematician, not a doctor
Body Mass Index was developed in the 1830s not by a physician studying individual health, but by Adolphe Quetelet, a Belgian mathematician and astronomer interested in applying statistics to human populations — a field he called "social physics." Quetelet wasn't trying to build a diagnostic tool for individual patients at all; he was looking for a formula that described the average build of a population for statistical and sociological study.
How it became a medical standard anyway
BMI stayed a niche statistical tool for over a century until the 1970s and 80s, when physiologist Ancel Keys popularized it (renaming Quetelet's formula "Body Mass Index" in a 1972 paper) as a practical, inexpensive way to screen for obesity across large populations using only height and weight — far cheaper and faster than more accurate body-fat measurement methods. Insurance companies and public health bodies adopted it quickly because of that simplicity, and it became the default screening tool almost by accident of convenience rather than because it was the most medically precise option available.
Why it doesn't fit everyone equally well
Because BMI is just a ratio of weight to height squared, it can't distinguish between muscle mass and fat mass. A muscular athlete can register as "overweight" or even "obese" by BMI despite having very low body fat, while someone with a "normal" BMI can still carry an unhealthy amount of visceral fat. BMI's population-level averages also don't perfectly account for age, sex, or ethnicity, all of which affect the relationship between weight, height, and actual health risk.
So why is it still used?
Despite its flaws, BMI remains useful precisely because of the same trait that made it popular in the 1970s: it requires no special equipment, takes seconds to calculate, and correlates reasonably well with health risk across large populations, even though it can be misleading for any specific individual. Most doctors today treat it as one screening data point among several, not a standalone diagnosis.
